What is the Escalated Care Pathway?

The Escalated Care Pathway (ECP) is a fully-funded ACC pilot pathway aimed at improving access to surgical and rehabilitative services for clients with neck and low back pain. The Rehab Team is excited to be one of a few practices in Tauranga, Papamoa and surrounding areas to provide this service.

Escalated Care Pathway services include conservative rehabilitation pathways, pre and post surgical rehab and return to work services. ECP is completed alongside a wider multidisciplinary team including Physiotherapists, Occupational Therapists, Surgeons, Psychologists, Dietitians and cultural advisors.

Who is this programme for?

The Escalated Care Pathway (ECP) is for people aged over 18 years with accident-related neck and low back pain lasting at least 3 weeks who have not resolved with primary care input. ECP is currently available for people living in the Bay of Plenty, Waikato and Lakes D would lHB regions.

The Rehab Team’s Tauranga and Papamoa based physiotherapists are able to provide this service to people living in Tauranga, Papamoa, Mount Maunganui and surround areas. As we also provide mobile, in-home physiotherapy services, we are able to travel to clients living in Te Puke, Whakatane, Opotiki and surround locations.

Who can refer to this programme?

Anyone can refer into this programme - this includes health providers as well as self referrals which are then screened to determine eligibility.
